This is a very real concern and you should have him assessed by a vet urgently. Infection, injury, heart and lung disease, parasites, clotting issues and foreign bodies could all be at play. A full physical and possibly chest xrays are recommended immediately

    Coughing up blood is a medical emergency and must be assessed by a vet immediately. Traumatic wounds to the throat, lungworm and other respiratory conditions can all cause similar signs and can be life threatening if not caught in time. It is likely sedation and xrays will be required if nothing is identified on an oral exam
