My cat is losing weight, confused, and has bloody stool. What's wrong?

Sophie was abandoned, I took her in 18 months ago. She has lost a lot of weight, sometimes doesn't use litter box. Seems to be confused as to where to go to the bathroom. Sometimes i have to walk with her to the bathroom or pick her up before she goes and take her. Spotted a small amount of blood in stool a couple of days ago nothing today. Litter is always clean. She's eats good and loves cold water. She's light as a feather.

I recommend taking Sophie to the veterinarian. Especially without knowing how old Sophie is it is difficult to determine the exact cause of the weight loss and confusion in her. At the vet, after doing a physical exam, they will most likely perform blood work to determine if Sophie has anything going on with her. There are a few different older cat diseases that can cause weight loss. Some older cat diseases can be anything from kidney disease, diabetes or even hyperthyroidism.
