My Shih Tzu is very sick and suffering. How do I know when to euthanize?

Pet's info: Dog | Shih Tzu | Female | spayed

12.5 yo shihtzu has MVD, on 6 meds 2x/day, 5/6 murmer, no fluid in lungs.Tiny interest in food (Inc chocolate i tried as test) past 2 dys;was able to get pills in her w/ marshmallow at nite. Drinking more H2O. Will call vet Mon. Am i wrong considering having her put down? Obv internal issue(lil diarrhea, 1x vomit; liver maybe?), hrt will get worse, won't eat so eating pills inconsistent.I LOVE her so much & am struggling but she has small quality of life & dont want her 2 suffer at all. Ideas?

You are not wrong for having her put down if she is suffering and you do not think she has A happy enjoyable life. If she cannot take her heart medicine then heart failure will set in quicker. You might could ask your vet about getting compounded liquid medicines to get in her better for a last effort. But if you feel in your heart it is time then you have to go with that
