Best milk replacer for young puppy taken from mother, where to buy?

Pet's info: Dog | Mixed Breed Small (up to 22lb) | Male | unneutered | 1 month and 21 days old | 4 lbs
I'm looking for a good milk replacement for a puppy that was taken from his mother too early and is soon going through deowrming. Are there any vets with recommendations and where I can purchase it? Walmart or tractor supply, maybe?
