My cat has neck hair loss. Why isn't his usual cream working now?

My cat has an area of hair loss in his neck it increases in size ,he had it befor and cured with corticosteriod antifungal cream but this time not responding well. Pet's info: Cat | Mixed Breed | Male | neutered | 4 years and 9 months old | 12 lbs
