My dog's tail sore came back, spreading, with a new painful bump. Why?

My dog had an infected sore in the crease (base of tail). Went to vet. They cleaned it and gave topical and oral meds. It got better but then started growing again. It's crusty, brown, painful, spreading. Today I notice a small grayish bump with reddening of skin around it. Also painful. What could this be? Pet's info: Dog | Silky Terrier | Female | spayed | 14 lbs

It sounds like he has a skin infection that needs to be treated again. Maybe he needed to stay on the antibiotics longer, either topical or oral.
