How can I move my 13-year-old cat to a new home with minimal stress?

I need advice on how to move my 13 y/o cat to my new home. I live about an hour and a half away from where he is staying now, with my mom. I cannot trust her to take care of him, as the last time I visited his coat had become matted and he had lost 3 of his front teeth. It is important for both his health and mine that he now moves with me.
