My tiny puppy has low blood sugar and lethargy. Is it a liver shunt?

Pet's info: Dog | Mixed Breed | Female | unspayed | 5 months and 3 days old | 3 lbs

We have 2 shorkie-poo puppies, litter mates/sisters who are 5 months old. One is 6 pounds and healthy and happy. Our other is tiny at just abput 3 pounds, sweet natured but has had blood sugar crashes and lethargy off and on since 4 mths. Had a bad week...lft's and ammonia very high. Acid bile test results terrible with pre number 380 and 2 hr post mid 400's. We have her on lactulose, flagyl and kd. We see radiology this week...any chance she won't have a shunt?

I am so sorry Claire is not feeling well. would recommend trying to give her something like nutrical every few hours to try and prevent her sugar from crashing low. It looks like the following blood work looks very suspicious of a liver shunt. Liver shunts are more common in small breed dogs like Claire. I would high recommend continuing to move forward with the radiologist to determine for sure if a shunt is present. Hope claire feels better!
