Dog scratching ear until it bleeds, vet said permanent. What can I do?

Pet's info: Dog | Mixed Breed Small (up to 22lb) | Female | 4 lbs

my dog keep scratching its skin after its ear. the dog acts this way till it blooded. i have consulted with the vet of the city and he told me that its permanent but he keeps doing this. what do you suggest me. what kind of medicine should i use and what do i have to do to make the dog stop doing that.

From what i can see in the picture it looks badly infected, this means that Bora must have an antibiotic course in order for it to heal, you should return to the vet in order to get it treated, without antibiotics this condition will not get better.
