Why is my sweet cat aggressive after being lost for a month?

Pet's info: Cat | American Shorthair | Male | unneutered | 1 year old | 12 lbs

About a month ago I went to stay with my grandpa and brought my 1 year old unneuterd Male tabby because he has Feline Leukemia and needed his medication. Well he went outside and disappeared for about a month. My grandpa called me today and said he was back but couldn't catch him (it is not like him to run from people) so I came over to catch him and he wouldn't let me near him, panicked, hissed and took off running. He is a super sweet and laid back cat before, what's going on?

Poor Sebastian. He could be scared or traumatized from being outside. He may have gotten into a fight while being on his own and he could be hurt. Sometimes when vets are hurt they will be skoddosh because they don't want people touching them. If there is a way a trap him and take him to the vet for an exam, you may want to do that. Just be careful to not get bit. Also you could try to offer him some cat food and treats and see if you can regain his trust. I hope he starts to come around for you.
