Dog has blood in urine, thick bladder, culture negative. Is it infection or cancer?

Hello, 1 year old male dog ultrasound showed bladder wall thickening, has blood in urine. Urine culture negative for bacteria. Vet's next step is tissue biopsy. Do bacterial infections always show in cultures? Is the only remaining diagnosis cancer?

I would recommend to treat medically before doing any invasive procedure such as tissue biopsy. Inflammatory type of cystitis are common and bladder wall can be often thicker than normal (I should see the ultrasound images though to give you an opinion on that). mAnti-inflammatory medications (i would associate an antibiotic anyway even with negative culture) can be tried before deciding any invasive test. With protein and blood present without an infection you will need more testing to be sure. An X-ray and ultrasound would be beneficial since a concern at this point would be bladder stones or bladder cancer. Both will show up on an ultrasound to determine what the next steps may be. The ultrasound will also check his prostate he is not neutered as this can affect the urine also
