My dog is thin and has small white worms again. What should I do?

Pet's info: Dog | Mixed Breed Medium (23 - 60lb) | Male | neutered | 20 lbs

I adopted a boxer/lab mix back in January, he is very thin. About 3 months ago I noticed he had worms, not knowing what kind of worms; I bought an over the counter worm medicine that worked for 3 different types of worms. It's worked for a while but I just noticed he has worms again, they are small and white. He is about 2-3 years old.

If they look like grains of rice it could be tapeworms which can be contracted from fleas. It could also be he is gotten reinfected if the eggs of the previous worms are in his environment such as in the backyard. There are over-the-counter flea medications that will take care of various intestinal parasites including tapeworms. However he should also be on a routine flea preventative to prevent re-infections.
