My lost dog is back, skinny, sick, not eating, and can't walk. Help!

I couldnt find my dog till today and i found her under the house and she was very skinny and didnt want to eat but was very thirsty she has a hard time walking she might of got bit by a spider but im not sure and i think she has worms what is wrong with her. Pet's info: Dog | German Shepherd | Male
