Puppy spay incision hard, swollen, pus in urine. Is vet advice right?

Pet's info: Dog | Mixed Breed Medium (23 - 60lb) | Female | spayed | 5 months and 2 days old | 15 lbs

Hi, I am living in W. Africa. I got my 5 mths old puppy spayed by a local vet 8 days ago. She had a follow up today he told me that the area around the incision feels hard. It is also still a bit swollen. Her urine has had the odd blob of something thick e..g pus in it. She has no pain/signs of infection around wound but he told me that it could be a fatty deposit that needs removing, and that he might have to open her up again if it is not gone in 48 hours. Would this be correct?

The whole story sounds a little weird to me, it just sounds as she might need an antibiotic course, this should resolve both problems. my only concern would be if the internal stitches did not hold, that is very rare and could cause a lot of problems of course, but i would definitely wait more than 48 hours unless some other more severe symptoms are showing.
