Why is my dog pacing and whining after new seizure medication?

Pet's info: Dog | American Staffordshire Terrier | Female | unspayed | 65 lbs

5 year old female dog has had seizures from a suspected poisoning. She's been on medication for the past 6 months. 2 tablets of 64.8mg of phenobarbital every twelve hours. Seizures were controlled until recently. Vet added 2 capsules of 300mg gabepentin every 6 hours to control the seizures again. Dog has been constantly pacing and whining and barking for a day and a half. Vet only suggests observation of the dog and isn't presenting with any other options. other possible options???

Based on the history, a toxicity seems less likely and a primary disease of the brain more likely. If the seizures are not well controlled on phenobarbital it may be that she needs a higher dose. You should discuss phenobarbital levels with your veterinarian. If the levels are in therapeutic range, a second anticonvulsant such as levetiracitam should be considered. Furthermore, you may consider following up with a veterinary neurologist to discuss diagnostics including an MRI of the brain.
