Cat with discolored nostrils, labored breathing, head shake. Is it serious?

I saw a cat today that had discolored nostrils and something sticking out of his nose his breathing was labored and he had a slight head shake getting up...I can't tell you too many details about the cat as he belongs to a girl I use to work with...but I wanna help that cat! Pet's info: Cat | American Shorthair | Male | neutered | 10 lbs
