My dog had a bad ear infection (diagnosed by vet) perforated ear drum and suspected vestibular disease. She is 10 years old. She received antibiotics and steroids but today has become unable to walk straight and doesn't seem to recognise anyone. What could this be?

The inability to walk strait could be related to the vestibular syndrome caused by the perforated ear drum and infection, this would not explain the second symptom though, which is more likely to be related to a brain condition and can cause the other vestibular symptoms too.
