These small black dots and missing hair could be caused by external parasites, such as fleas and mites, allergies, or illness. Itchy ears could be cause by ear mites or an ear infection. The only way to know for certain what is causing these issues in Lilly is to take her into your vet for a health check. I'd recommend taking her in to see your vet to get an exact diagnosis and correct treatment.

If she has been scratching and chewing at these areas, the small black dots may be increased pigmentation of the skin - they may appear as black specks or blotches, and the skin in this area may also be somewhat dry or thickened. If this is the case, the discoloration is nothing to worry about - just a side effect of all the scratching. But, I would recommend having her checked out by a vet just to make sure there is no problem. He/she can also help you with the itching problem - itchy skin and ears are most often allergy-related in dogs.
